Program Description
By providing verbal consent at the point of care, you agree to receive text messages and/or emails from Chroma Dental regarding follow-up on dental treatment recommendations discussed during your visit. This program is operated by Chroma Dental, located in New York, NY.
Eligibility
This messaging program is available to patients of Chroma Dental who have provided valid contact information and verbal consent to receive communications.
Message Frequency
Message frequency varies. You may receive up to 6 messages per treatment type over a period of up to 60 days following your visit. Messages are sent via SMS and/or email depending on your consent preferences.
Supported Carriers
Supported carriers include but are not limited to: AT&T, T-Mobile, Verizon, Sprint, US Cellular, and other major US carriers. Carrier coverage may vary.
Message and Data Rates
Message and data rates may apply. Chroma Dental is not responsible for any charges incurred from your mobile carrier in connection with receiving messages.
Opt-Out Instructions
You may cancel SMS messages at any time by texting STOP to any message received from Chroma Dental. After texting STOP, you will receive one final confirmation message confirming your opt-out. No further messages will be sent unless you re-enroll. To opt out of email communications, reply UNSUBSCRIBE to any email from Chroma Dental.
